Survey: The Long-term Plan - Closing Date Friday 31st May 2019

Local Healthwatch services are supporting us in finding out what's important to you and your families, so that we can shape services that meet your future needs.

They will be visiting groups across the Frimley Health and Care Integrated Care System to speak directly with local people to find out what matters to them.

There is also an online survey available for you to be able to share your views.

The information you provide will be shared anonymously with local health and care services and a report published on this website and the sites of local Healthwatch services in July 2019.

Survey: Community Services

This survey has now closed. 

We are reviewing local community services to see where we can make improvements. This is particularly important as in recent years we have been working to help support people in or near their homes and away from acute hospitals.

Community services include most services that fall between a GP surgery and a large acute hospital, usually provided in community hospitals, local clinics or in the patient's home.

They can include: 

  • nursing care for wound management and medication management
  • support for people with long-term conditions from a team of different health and care practitioners with a wide-range of skills and knowledge
  • specialist support for specific conditions such as dementia and multiple sclerosis (MS), and care for people in nursing homes
