The 2018 Making a Difference Awards was the first of its kind, bringing together over 150 members of Surrey Heath’s health and social care teams to celebrate the achievements of Integrated Care across health, social care and General Practice.

96 nominations were submitted in seven categories, each one truly reflecting the outstanding commitment staff make in supporting the health and wellbeing of residents in Surrey Heath. The shortlisting team were honoured to read every single nomination and hear first-hand about some of the exceptional work that happens day-to-day on the ground.

Surrey Heath Clinical Commissioning Group was delighted to welcome Professor Jane Cummings - Chief Nursing Officer England and Ruth May - Executive Director of Nursing, NHS Improvement who attended the event and presented each winner with their certificate.

Jane and Ruth began their day with a visit to Surrey Heath’s Single Point of Access to meet the Surrey Heath Integrated Care team comprising of professionals in community nursing, voluntary services, specialist nurses, mental health, adult social care and community rehabilitation. The Integrated Care team works as one service to ensure residents and their carers are supported at home where possible, in turn reducing hospital admissions.

Jane and Ruth then joined the community team on a home visit followed by multi-disciplinary meetings at Lightwater and Upper Gordon Road practices before attending the awards ceremony in the afternoon.

Dr. Andy Brooks, Chief Officer of Surrey Heath CCG comments, “These awards are a true reflection of the outstanding work that goes on across Surrey Heath.
